If you do not use insurance, you have the right to a Good Faith Estimate. Ask us, and we give it to you in writing before your visit. X-rays are taken only when medically necessary, and we tell you what they cost and show them to you first. We do not bill insurance.
Receipts and claims
What a superbill can show, line by line
Ask for one at any visit.
- 01
Step 1: Your receipt at the visit
You pay at the front desk and get a paid, itemized receipt the same day.
- 02
Step 2: What a superbill can show
It can be the record your plan asks for.
The date of each visit
Each service and its code
Our fee for each and what you paid
The doctor's name, license and identification
- 03
Step 3: Your plan's claim form
Most plans post an out-of-network claim form on their website or member portal. We can answer questions about the superbill, and the plan decides what it reimburses.
- 04
Step 4: Send it in
You can send the superbill with the form, by the plan's own method, and keep a copy.
- 05
Step 5: The plan decides
The plan decides what it reimburses, and how much. We cannot tell you in advance what that will be.
How fees work
If you pay yourself
You pay at the visit, line by line. What sets the fee is which lines your visit has, from the ledger above. The front desk tells you each line's fee in your written estimate, and your doctor tells you the cost of care before it begins.
Receipts for reimbursement
Every visit can end with an itemized superbill. It shows the date, each service and its code, our fee for each, what you paid, and the doctor's license and identification. You send it to your plan with the plan's own claim form.

If you do not use insurance, you have the right to a Good Faith Estimate. Ask us, and we give it to you in writing before your visit: each line, its fee and what the visit includes.
Insurance we work with
We are out of network, so we do not bill your plan. A plan may reimburse part of an out-of-network visit, all of it or none, and the plan decides.

Checking your benefits
Before your first visit, call your plan and ask what it reimburses for out-of-network chiropractic care. Our front desk will help you read the answer, but the plan decides.
What the first visit includes
A talk, a hands-on exam, an explanation and a written estimate. Plan on about 45 minutes. X-rays only when medically necessary.
Your first visit
Two more answers
X-rays
X-rays are taken only when medically necessary. If your doctor recommends them, you hear why and what they cost before they are taken, and the cost can be on your superbill.
If you cannot make a visit
Call as early as you can so another patient can have your time. We do not charge a fee for a missed visit.
Ask for your estimate
The front desk gives it in writing before your visit.
